Meet Dervene – RPM Care Coordinator
Dervene is a dedicated Remote Patient Monitoring Care Coordinator who works closely with patients enrolled in the RPM program. She plays a key role in supporting patients, reviewing incoming health data, and ensuring concerns are communicated promptly to the clinical team.
Dervene helps patients:
● Get comfortable using RPM devices
● Stay engaged and consistent with daily readings
● Understand their care plan and next steps
● Feel supported between doctor visits
